Patient Representation Group
We are currently in the process of recruiting patients to our Patient Representation Group.
If you would like to become a member of this vital group and help steer the surgery, please contact the surgery in person, or use the message window on our Contacts page.
The aim of our PRG group will be to improve communication between the Medical Centre and patients to ensure the services on offer truly meet the needs of all patients.

Welcome to Laurie Pike Health Centre
There are two reserved car parking spaces clearly marked for disabled patients. The entrance to the building is via dropped paving stones and automatic doors to permit access for wheelchair users.
An induction loop operates within the Practice telephone system to assist those with hearing difficulties.
All patient services are provided at ground floor level and a disabled patients'
WC is provided to the right of the waiting room. Please let us know if you have any
difficulties with access.
How Do I… Contact The Practice?
You can contact the practice by telephone, post and fax at any time for repeat prescriptions or to inform us of changes to your details.
Phone lines open at 8.00am but can be very busy; we would therefore ask that for routine appointments and enquiries you call after 10.00am.
How Do I… Obtain A Home Visit?
Home visits are provided for patients that are housebound or too ill to attend the surgery.
Please telephone the Practice before 10.00am to request one, and do give the receptionist as much information as possible to enable the doctor to allocate priority to visits.
Remember that clinical examination is best carried out at the surgery.
Please note: lack of transport shall not mean a home visit is appropriate.
How Do I… Make An Appointment?
Routine Appointments
All patients with routine medical needs are seen by appointment
only. When booking an appointment, please let the receptionist know if you have any
preference as to which practitioner you see. Patients are free to make an appointment
with any of the doctors or nurses but are encouraged to see the same person for follow-

HOW DO I… Obtain A Repeat Prescription?
If it has been agreed that your treatment is long term, you may not need to see your
doctor every time you need your medication. The doctor will then approve certain
medication to be issued on a repeat prescription basis.
When ordering repeat prescriptions
please remember to include the patient's full name, date of birth and address details,
medication required and a contact telephone number.
Please note you will not receive
your prescription immediately. All requests received by 2.00pm will be available

Out Of Hours
If you are suddenly taken ill outside the practice opening hours, please call the
practice on 0345 1111 1312 and you will be transferred to the out-

Your details will be taken for a doctor or nurse to call you back to advise you.
Depending on the nature of the illness, you may be asked to attend a centre near to you, for treatment.
This number can be obtained by calling the practice number at any time outside opening hours.
Have a pen and paper ready and a message on the answering machine will give you instructions on how to contact a doctor.

Access To Information Rights In Relation To Disclosure
As a patient you have the right to access your medical records which can only be done in the presence of a doctor or a nurse.
You are able to obtain copies of your medical records, for which a fee is payable as per the BMA (British Medical Association’s) recommendation.
Other services, such as social services, solicitors, insurance companies and other medical professionals can also ask to obtain medical information from your records.
The information required can only be disclosed to these other services if you provide them with a signed consent which needs to be sent to us before we can act.
Complaints Procedure
The partners and staff at Handsworth Wood Medical Centre recognise that occasionally,
everyone makes mistakes -
If you have a complaint or concern about the service you have received from the practitioners or any members of staff in this practice, please ask for a complaints form from reception and the operations manager will deal with this.
We will endeavour to resolve problems as soon as possible.
Zero Tolerance
The practice supports 'Zero Tolerance' and any patient behaving in an inappropriate, abusive, violent or intimidating way towards practice staff, community staff or other patients, will be removed from the practice list. This removal will be effective immediately where a member of staff or patient has felt threatened.
Notification of removal will be issued in writing except in cases where it is felt that by doing so the practice would be endangering itself.

Medical Training
Our practice is committed to the teaching and training of healthcare professionals. From time to time you may find doctors, medical students and student nurses will be sitting in with the doctors and nurses at the practice to learn about General Practice.
Please remember that Medical Students are bound by the same confidentiality rules as all other practice staff. Should you wish to see the doctor or nurse alone, please feel free to say so.
For training purposes, you may find our doctors and nurses videoing your consultation. You will be asked to sign a consent form to allow this video to be used for assessment purposes only. Again, you do not have to give your consent and can ask that the session is not recorded. You can also ask for the recording to be stopped at any time.
Practice Nurses
There are five practice nurses who run various clinics, deal with immunisations, the removal of stitches, dressing of wounds, blood tests and other investigations ordered by the doctor.
Birmingham City University Nurses
There are three BCU nurses based at Perry Barr, Westbourne Road and Gosta Green sites.
They run a number of clinics to support students’ needs at these sites.
Health Visitors
The health visitors can be contacted between Monday and Friday on 0121 255 9301.
District Nurses
There is a team of district nurses attached to this practice. They may be contacted on 0121 465 5444.
Physiotherapist
A physiotherapist works at the practice. If appropriate, the doctor may refer you to a physiotherapist.

About us
The Practice, originally known as Birchfield Medical Centre, was established by Dr
Pinsent and Dr Pike in the 1960s. In 1977 the Practice relocated to its present site
and in 1990 was renamed the Laurie Pike Health Centre in honour of Dr Pike on his
retirement. The Practice is now run as a non-
As well as offering a wide range of NHS services, the centre operates as
a training practice for undergraduate and postgraduate education and participates
in research projects in association with the University of Birmingham, Department
of Primary Care and General Practice.
In 1999, the Practice successfully applied to
become a Personal Medical Services (PMS) pilot site, which enabled the Practice to
be more flexible in providing specialised clinics and associated services. This flexibility
is extended under Practice Based Commissioning and The Laurie Pike Health Centre
is part of the One Stop Commissioning Group, a number of local like-

How Do I.… Register With The Practice?
All patients applying to register with the practice must provide evidence of entitlement to NHS treatment.
Where a prospective patient is entitled to receive NHS care, a registration form must be completed.
You can download a GMS1 form OR you can complete online HERE.
You will be asked to complete a short health questionnaire when you apply online or arrive at the surgery.
You are required to book a registration medical appointment either by telephoning the practice you are registered to or by attending the practice. On the day of your registration medical you will need to bring an early morning urine sample.
If you wish to be registered at a Vitality practice please provide the following
:-
• Complete and sign the purple GMS1 application form
• Complete the medical questionnaire and the patient profile
• If female provide a copy of your latest cervical smear test
• If registering a child, we will need a copy of the vaccinations page of the child’s Red Book at your first visit.
• Copies of a UK passport or photo driving licence or valid NHS medical card
Additionally, if you are NOT a British citizen you must provide :-
• A copy of your valid national passport and evidence of your right to remain in the UK
• Proof that you have been in the UK for 6 months or over (utility bill addressed to you and older than 6 months) or that you have the right to remain.
• Proof of address (e.g. letter from Home Office addressed to you)
• Proof of full time work or full time education
If you are an ASYLUM SEEKER you must provide :-
• An HC2 form
• Your identity card provided by the Home Office
When you have completed ALL of the above please make an appointment for a Registration Medical. If you bring the originals of the above documents we will make the photocopies.
If any of the above has not been done you will NOT be allowed to register and any treatment can only be given as a private paying patient. If you CANNOT SPEAK ENGLISH please bring someone with you who can TRANSLATE.
